Service Description
ADHD Screening Evaluation i The ADHD screening appointment is approximately 45 minutes in length. This evaluation is preformed the session following the completion of the bio-psycho-social assessment and after the necessary forms have been submitted for reviewed. *Please note, this differs slightly from Neuropsychological testing that is preformed by a Psy.D. or Ph.D. level psychologist. At least 72-hours prior to the appointment the patient is to fill out and submit all of the ADHD Screening Forms in full so they can be uploaded into the necessary documentation at the time of your session. If this is not done, one may need to complete a 2nd add on session in order to achieve completion of this assessment. If during this appointment, education and discussion of treatment and diagnoses takes last through the end of the appointment slot, an additional appointment may be scheduled to conclude the discussion and potential prescribing of medications. Note, that one may leave this appointment without an ADHD diagnosis and without medications being prescribed. *Please be aware that controlled substances / scheduled medications will not be prescribed at this time, however there are FDA evidence-based non-stimulant medications that can be discussed further if one meets criteria. The option for stimulant medication may be an option after establishing a working relationship and the controlled substance agreement has been completed. Fee For Service Psyched is not a participating provider on any health insurance plan, nor does Columbus Psychiatry file health insurance claims. Payment is due before the appointment. At this time Psyched only accepts electronic pre-payment in full for services. A receipt can be provided, upon request, at the end of the visit for submission for out-of-network claims.
Cancellation Policy
Cancelation Policy 72-hour Reschedule (1-Week Cancellation) Policy Last minute cancellations/rescheduled appointments and "No-show" appointments have a negative impact on a business, including lost wages for our staff. In addition, when you forget or cancel your appointment without giving enough notice, we miss the opportunity to fill the appointment and patients on our waitlist miss the opportunity to receive services they need. For this reason and out of common courtesy to our providers, Psyched has the following Cancellation and Reschedule Policy: A non-refundable deposit is due at the time of booking. The remaining service fee will be charged one week prior to the service date. Once processed, these payments are non-refundable. Psyched understands things happen, thus if you need to reschedule, be sure to address this more than 72-hours (6:59 PM EST) prior to the appointment date and this can be credited towards any future service(s) executed within 90-days of the original appointment date. Credits are not guaranteed for cancelled or rescheduled appointments less than 72-hours prior to the appointment date and will not be arranged for any “no-show” appointment. For services with variable pricing, the lowest price will apply, i.e. 20 unit minimum for Botox, smallest syringe of filler or single vial for Kybella or sclerosant *these services are currently on hold. Excessive cancellations/rescheduled appointments may require full payment before appointments will be confirmed. Special discounts and pricing are not valid if this policy has been breached. By scheduling an appointment, you are agreeing to our cancellation policy. For those with unpredictable schedules, we do offer concierge appointments which can be requested through the contact page or email listed on our website. Your time is just as valuable to us as our own. As a courtesy, reminders will be sent to the email and phone number you provided when scheduling, however, if you do not receive a reminder, the above policy remains in place. We understand that emergencies can happen. Kindly notify us by email should an unforeseen event prevent you from keeping your scheduled appointment. Thank you, Psyched
